SEC Supplemental Benefits Program

This combined synopsis solicitation is amended to add four (4) attachments to the Request for Proposal (RFP).  The attachments are:

•  Attachment 7 - Contractor Data Form V.3
•  Attachment 8 - 0F306 Declaration for Federal Employment
•  Attachment 9 - Credit Release Form
•  Attachment 10 - Fingerprint Form

All other terms and conditions remain unchanged.

SEC Supplemental Benefits Program (SBP) shall include dental and vision plans, medical evacuation insurance, and other voluntary benefits for its employees.
