R499--Community Living Centers (CLC) Long Term Care

The Department of Veterans Affairs (VA), Veterans Health Administration (VHA), Geriatrics and Extended Care (GEC) Program Office has a requirement for conducting surveys at VA owned and operated Community Living Centers (CLCs) and Spinal Cord Injury/Disorders (SCI/D) Long Term Care Centers to be performed from an independent survey company/contractor.
