Please NOTE:

• One (1) Contractor Request for 1-year contract to include: Base (6-months) plus 4 Option Periods (Each 6-months). 
• Two (2) Contractor Request for 6-months only to include: Base (6-months only).

**It is the responsbility of the Offeror to submit only qualified, applicable, and available key personnel. Any any key personnel submitted by multiple Offerors will not be forwarded for evaluation and further consideration.**
