AMENDMENT 0004- The purpose of this amendment is to provide responses to vendors' questions.  The following responses are provided:  (SEE ATTACHMENT)

1. Question 1:  Place of Performance (pages 2 & 3):

Please confirm that there are two separate locations where services are required:

• Eugene J. Towbin Healthcare Center: 2200 West Fort Roots Drive North Little Rock, AR  72114.
• John L. McClellan Memorial Veterans Hospital: 4300 W 7th St. Little Rock. AR 72205.

AMENDMENT - The purpose of this amendment is to extend the due date and time for responses from 8:00 AM 1/27/26 to now read 10:00am 2/3/26.

AMENDMENT - The purpose of this amendment is to incorporate vendors' questions and Government answers in this solicitation.

The following answers/response are provide for vendors' questions.

Question 1: Can you provide a list of devices that require calibration?
Answer 1: Please refer to the Performance Work Statement (PWS). The PWS includes all
devices that need calibration.

Question 2: Can you please confirm that there is no up time required between safety device
testing and the tuning? They could occur during the same semi-annual visit.
Answer: Yes.

AMENDMENT - The purpose of this amendment is to incorporate a site visit for this solicitation.

All contractors are highly encouraged to attend the site visit to be held on:
