The purpose of amendment 0003 is to update Lost Containers description included in the Pricing Spreadsheet.
"LIQ" has been replaced with "OxyTOTE"
Please see the updated Pricing Spreadsheet titled "Pricing Spreadsheet-SYR GAS_Final Updated".
Also, attached to this amendment 0003, on page 2 are the Loss Container categories each CLIN fall under.

All other terms and conditions of the solicitation remain unchanged.

The VHA, VISN 2, Syracuse VA Medical Center require a contractor to provide medical gases to the VA medical center facilities. The Contractor shall be responsible for providing, filling, transporting, handling, servicing, maintaining, delivering, picking up and storing medical gases and cylinders for all facilities located within VISN 2, Syracuse VA medical center and VA Dental clinic catchment area.
